Credit Application
Companies must be approved prior to ordering by purchase order. Federal, State, and Municipal Agencies including Educational Institutions, Hospitals, and Fortune 500 companies are pre-approved to use purchase orders, understanding that payment terms are NET 30 days after date of invoice. Other companies may be approved to be put on these terms after completing a credit application.
The easiest way to fill out a credit application is to print it out and fax it back to us. Please note you will need Adobe Acrobat to print out the credit application. We can also fax or email you a credit application. Simply call us at 866.975.4846 or email corporate@LivingDirect.com. Please make sure to fill out all of the information on the credit application. Failure to do so may result in delay of your order.
